Originally established as a residency program, the need for specialized geriatric care in rural Wyoming soon became undeniable. As the state’s population aged and traditional healthcare models proved insufficient for high-risk elderly patients, the clinic reimagined its mission by integrating comprehensive geriatric care with cutting-edge education.
Recognizing the gap in services for aging residents, the clinic transformed its approach by leveraging existing educational infrastructure to create a dedicated geriatric ecosystem. Through innovative Fellowship and Clinical PharmD Residency programs, Wyoming Family Practice delivers exceptional care and trains the next generation of geriatric specialists. This integrated model allows board-certified geriatricians, advanced practice providers, clinical pharmacists and case managers to work as a seamless team, addressing complex geriatric issues that range from medication management and cognitive evaluation to fall risk and social determinants of health.
At the heart of the geriatric department is the specialized Geriatric Assessment Team, a multidisciplinary unit designed to evaluate high-risk patients comprehensively. By integrating services across outpatient clinics, nursing homes, hospitals and even patients’ homes, the team ensures continuity of care that other systems lack. Strategic partnerships with organizations and participation in the federal GWEP grant via WyCOA underscore their commitment to transforming geriatric healthcare not just locally, but as a model for rural regions nationwide.
